    In consideration of payment being made in accordance with the terms set forth in the contract, ONSITE SOLUTIONS OF VIRGINIA, INC., (Onsite Solutions) agrees to provide the monitoring, sampling and reporting activities outlined in the contract.

    General Description of Payment Terms

    In order to provide more convenience to our clients, ONSITE SOLUTIONS offers two Payment Options (described below in Section C). If the client chooses to enact this agreement, payment will be due before the contract can be executed. If Option 1 is chosen, payment will be due at each anniversary of the execution of this contract. If Option 2 is chosen, payment will be deducted on the same day each month, every month until the contract term is fulfilled, or contract is terminated in accordance with the terms outlined in Section 9 of the contract. If Option 2 is chosen, please be sure to complete and sign the bottom of this page.

    Rebate/Refund Information

    If any terms of this agreement, or related articles, are violated, the contract will be immediately terminated, and no rebates or refunds will be issued. If this agreement is terminated or canceled in accordance with the guidelines in Section 9 of the contract, a pro-rated refund or rebate may be issued, if deemed by ONSITE SOLUTIONS to be necessary and worthwhile.

    Payment Options

    Option 1: The client may choose to pre-pay for the entire year of service. If this option is chosen, full payment of the total yearly sum will be required prior to the beginning of each 12 month term. An invoice will be sent in accordance with section 4 of the contract.
    Option 2: The client may choose to pay in equal monthly installments, which will be deducted directly from clients' bank account or credit card every month for the term of the agreement. ONSITE SOLUTIONS OF VIRGINIA, INC. will deduct the money on the same day of each month.
